To help us make sure that you are prepared for your appointment, please fill out the necessary patient forms. Upon arrival, please register at the front desk. Personal and insurance information will be verified. If this is an initial evaluation, fill out a New Patient Form and be prepared to bring the following information to your appointment.

• Drivers License (or equivalent identification card)
• Your Insurance Card
• Any X-ray, CT, MRI reports related to your consultation
• Referring physician notes leading up to the consult
